This thesis was commissioned by a federation of municipalities in southern Finland. The aim of the thesis was to map out nurses’ experiences of cooperation between the home hospital and 24-hour service housing. Theory knowledge was looked for in information channels. The central concepts in the thesis were home hospital, 24-hour service housing and cooperation.

The working method in the thesis was qualitative research. Material was collected by using the theme interview as individual interviews and a group interview. The material was analyzed by using deductive and inductive analysis.

The cooperation between the home hospital and 24-hour service housing was versatile and varied. In the cooperation different conventions and varied realizations occurred. The conclusion is that the organization would benefit by creating a common procedure for cooperation. Challenges in coordination and communication during the cooperation also spur to form the procedure. In forming the procedure the organization could benefit from the already existing factors that promote cooperation.

Further study could be to examine the cooperation after the procedure has been implemented. Also experiences of the cooperation from the viewpoint of the patient and the relatives could be examined.
