Purpose of this thesis is to examine HaiPro-notifications of one wellbeing services county from the period that the commissioner of this thesis chose. HaiPro-notifications were organized by the type of the deviation. After this, the medication deviations was compared to the unit's pharmacotherapy plan. In the comparison, attention was paid to risk identification and protective mechanisms. The aim was to increase the safety of pharmacotherapy in the home care unit.

Background of this thesis deals with home care and pharmacotherapy from the point of view of customer safety, HaiPro-system generally and its use, as well as considering its safety in terms of pharmacotherapy. Identification of risks in pharmacotherapy, protective mechanisms and a pharmacotherapy plans in home care are also discussed. The thesis was carried out as a quantitative and practice-based thesis. HaiPro material was used for quantitative research. The results of quantitative research and background discussion were utilized for the practical part, in which a department hour was organized to discuss the most typical causes of medication deviations in home care units and how these drug deviations could be prevented.

At the end of the department hours, the employees gave written feedback. Based on written feedback, employees felt that the discussion on the topic was needed. The content of the department hour was considered informative and well-organized by the employees. Based on the feedback, it can be deduced that in the future, employees will pay more attention to carefulness, concentration, duplication check, communication, and up-to-date list of medicines in the implementation of pharmacotherapy. As a development proposal for the unit's home care, it is recommended that the HaiPro-notifications should be regularly reviewed.
